If you are a new patient and have an appointment scheduled, the new patient forms are provided below for you below to print/fill out prior to your appointment.  We need only one Patient Info and HIPPA Consent or HIPPA Opt Out per family with the insurance/financial info.  Each individual patient will require their own Medical and Dental Information forms.  If you are unfamiliar with HIPPA, we send claims electronically and must inform you of this per government regulation.  You can learn about these regulations by reading HIPPA Information.  Having these done ahead of time can really speed things up for you, but you can also complete them at the office if you prefer.
